The phrase "living proof of a dream's design" is a poetic way of saying that a person, an achievement, or a reality is the physical, breathing manifestation of an original vision or prayer.
 
It  speaks to the bridge between the invisible and the visible. We all have designs (goals, hopes, sketches of a better life). When those things finally happen, you aren't just lucky - you are evidence that the design worked.
